SUPPORTS COORDINATION

At PA Health Management, we aspire to bring support and direction to individuals across the Autism Spectrum and their families. We believe in establishing a personalized plan and including supports preferred by individuals and their families to help reach personal and developmental goals while setting an overall goal to increase self-sufficiency, improve quality of life and reduce stress.

PA Health Management is a Supports Coordination Provider for the Adult Autism Waiver (AAW) authorized by the Bureau of Autism Services (BAS), Office of Developmental Programs (ODP), Department of Human Services (DHS). Supports available are offered by the Adult Autism Waiver and are specialized for individuals across the wide array of the autism spectrum.

Supports Coordinators work with individuals and their families in several aspects:

  1. Develop an Individual Service Plan (ISP) centered around the individual’s unique needs and goals and communicate often to evaluate progress towards goals.
  2. Conduct assessments each year before developing the Individual Service Plan (ISP):
    • The Scales of Independent Behavior-Revised (SIB-R);
    • The Parental Stress Scale (PSS), if the individual lives with a caregiver; and
    • The Quality of Life Questionnaire (QOLQ).
  3. Assist individuals in locating and accessing services which may not be offered by the Adult Autism Waiver (AAW).
  4. Communicate, monitor progress and ensure the individual’s team meets to reevaluate effectiveness of the Individual Service Plan (ISP). Upon approval to receive waiver services, a wide variety of services become available to the individual. A Supports Coordinator will call at least once per month and visit in-person at least once per three months to ensure the individual is doing well and that their services are being delivered in the scope, duration and frequency as outlined in the plan.
