APPLYING FOR WAIVER SERVICES


Waiver services are available to eligible individuals as an option to assist in living safely in a personal home or in the community instead of moving to a nursing facility. When eligible, the State will "waive" funding for a nursing home and spend the funds instead on assisting an individual to live in the community, these services are called Home and Community-Based Services (HCBS). Home and Community-Based Services (HCBS) include long-term services and supports (LTSS) which are offered through several State waivers. Each waiver has different eligibility requirements and varying services that they offer.

PA Health Management can assist individuals who are interested in learning more about and applying for waiver services. It is important to take time to gain knowledge in options citizens have besides living in a nursing home, and we are available to share our knowledge and experience in the waiver application process and waiver service delivery. A list of waivers for receipt of services in the home and community is included below, along with their eligibility requirements and services provided. The Community HealthChoices (CHC) Waiver is currently available to eligible individuals in the Southwestern and Southeastern Pennsylvania Counties, detailed below, and will be offered state-wide on January 1, 2020.

SERVICE AREA

PA Health Management provides Service Coordination services in all 67 counties of Pennsylvania and Service Coordinators live locally in the areas they provide service to. The Community HealthChoices (CHC) Waiver is currently available to eligible individuals in the Southwestern and Southeastern Pennsylvania Counties. The Aging Waiver, Independence Wavier, Attendant Care Waiver, COMMCARE Waiver and OBRA Waiver are currently available to eligible individuals in the Lehigh/Capital, Northwest, and Northeast Pennsylvania Counties. The ACT 150 Program is offered state-wide. A complete list of counties covered is listed below.

MANAGED-CARE ORGANIZATIONS

The Department of Human Services has selected three Managed-Care Organizations (MCOs) to deliver Managed Long-Term Services and Supports (MLTSS) through the Community HealthChoices (CHC) Waiver. The CHC waiver began in Pennsylvania on January 01, 2018 and will be offered across the entire state of Pennsylvania by January 01, 2020. All individuals approved for the CHC waiver will be offered a choice of the following Managed-Care Organizations (MCOs) and individuals are able to change their MCO at any time.
